Pyridoxal 5’-phosphate (P-5-P) is the biologically active form of vitamin B6. It is the cofactor for at least 180 enzymes responsible for an extensive range of functions, including neurotransmitter production, heme biosynthesis, and the metabolism of glucose, lipids, amino acids, DNA, and more.

Although B6 refers to any of six different compounds, including pyridoxine, they each must be converted to the active form, P-5-P, to be used in enzymatic reactions.

The wide range of enzymes that require P-5-P as a cofactor (approximately 4% of all enzymes) may explain its strong inverse relationship with diverse chronic diseases.

For example, many inflammatory conditions have been associated with reduced plasma P-5-P levels, and an inverse relationship between C-reactive protein and P-5-P has been observed in several populations.

Plasma P-5-P also appears to modify the detrimental effect of blood lead levels on cardiovascular disease, as an increased risk was only found among participants with lower P-5-P.

Considerable genetic variability exists in the enzymes that convert other forms of B6 to P-5-P. This includes common variants in tissue non-specific alkaline phosphatase (ALPL) and the thymidylate synthase gene (TYMS), which lead to reduced P-5-P levels.

Supplementation with P-5-P not only bypasses potentially variable conversion steps, but supplementation with pyridoxine may competitively inhibit P-5-P, interfering with B6 functions.
